1. Please call 02 9912 4490 (during business hours) at least 3 weeks after but within 60 days from the date of purchase (inclusive of 60th day) to obtain a RETURN AUTHORISATION NUMBER

3. Send the form along with your Philips product (in its original packaging) and original purchase receipt (please retain a copy of the receipt) within 90 days of purchase (with final close date for claims being last mail on 30th May 2018) to:
